Care Coordination Roundtable

Roundtable discussion by care coordination experts discussing the use of care coordination strategies across national hospitals, what its definition is, what its role is in the hospital setting, how to create care coordination synergy across service lines and much more.

High level rationale for the discussion/sample questions roundtable members will discuss:

  1. There are so many different versions of care coordination going on across the United States. Some call it navigation or care coordination, some call it care management.  What is the difference, in your mind?
  2. Is navigation disease specific? Is care management just the organization of general patient activities like discharge, education, etc and when you add the layer of a care plans or algorithms for a disease - that makes it care "management"?
  3. Many hospital are just renaming their case management departments as care coordination - is this the right way to go?
  4. Is there any feasible reimbursement REALLY being leveraged by hospitals for care coordination activities such as Chronic Care Management or Transitional Care Management?

With over 30 years in the healthcare arena, he has developed significant experience and knowledge in this industry. He has a special interest and expertise in population health management, clinical care process design, cost accounting and hospital – physician integration.

Kim Zukowski is Director of Care Coordination for WellStar Health System. She defines the guidelines for and leads the 42-member care coordination and case management team serving patients and clients of WellStar’s Cobb Hospital. She also serves as a liaison and care coordination expert to medical staff, payors and hospital administration. She has worked in care coordination for WellStar since 2009. Previously, she worked as a case manager for HCA’s Cartersville Medical Center for six years, responsible for utilization review and discharge planning for up to 50 patients daily. She holds an MSW from the University of Georgia and graduated Summa Cum Laude from Georgia State University with a BS in Psychology. She is certified case manager.

Stefani Daniels, RN, MSNA, CMAC, ACM

Founder & Managing Partner, Phoenix Medical Management, Inc.

Stefani Daniels is the founder and managing partner of Phoenix Medical Management, Inc. She is a former hospital executive, chief nursing officer and senior consultant with Johnson & Johnson. Stefani held faculty appointments at University of Pennsylvania, Nova Southeastern University in the Graduate Division of the School of Business and Entrepreneurship and Columbia University in New York. She serves on the editorial board of Lippincott's Professional Case Management journal, HCPro's Case Management Monthly, and CMSA Today, and is a popular speaker at international, national and regional conferences. Stefani is a contributing author to the 2nd and the recently published 3rd edition of CMSA's Core Curriculum and is the co-author of The Leader's Guide to Hospital Case Management and the popular text The Hospital Guide to Contemporary Utilization Review.
