Three Reasons to Be Excited About Journey Connections

Cancer takes its toll both physically and emotionally on patients, as well as their friends and family. That is why a holistic approach to care leads to better outcomes – and why care coordination is so important. It helps people with cancer deal with factors beyond treatment, including practical matters (like finances and transportation) and the emotional support that is necessary for full recovery.

That’s why we’re so excited about the recent addition of Lilly Oncology’s Journey ConnectionsTM program to the Cordata Oncology specialty care coordination suite.  As we mentioned in the announcement of the collaboration, Journey ConnectionsTM is designed to connect cancer patients and their loved ones with local resources and programs that can help ease the burden of treatment. Lilly Oncology clearly shares Cordata’s vision for treating the whole patient.

Personally, the three things that I think make the Journey ConnectionsTM program so special and beneficial are:

  1. Comprehensive Care Resources
    There are so many things that people with cancer have to think about, from home and family care, to transportation, to financial assistance. Journey ConnectionsTM helps patients and their families find resources solutions for many of them. In fact, the program covers 12 different resource categories, which means easier access to a wide range of support options and a better overall experience for patients as they seek to recover.
  1. Personalized Support Plans
    Every person with cancer is on a unique journey. Journey ConnectionsTM helps coordinators provide individualized support. The database allows nurses and other healthcare professionals to create personalized plans with the specific emotional and logistical support resources patients are looking for.
  1. Knowledgeable and Connected Community
    At the highest level, what I like about Journey ConnectionsTM is that it keeps the big picture in mind. There’s a lot for each of us to learn in terms of improving cancer care and helping people with the disease, and Journey ConnectionsTM is designed to engage everyone involved in cancer care, from physicians to patients. When a new resource is added to the database, other users can access it, which helps to create a community of knowledgeable caregivers who can share what works.
