AdventHealth System’s Florida Hospital and its regional orthopedic leaders have chosen the Cordata specialty care coordination platform to coordinate orthopedic care and support patients at the hospital in its central division and south region, which incorporates the greater Orlando area.

Orthopedics is the third disease institute in that region to adopt Cordata’s specialty-specific platform and toolsets. AdventHealth’s cancer and neuroscience institutes have used Cordata since 2016.

Cordata’s platform features a first-of-its-kind care coordination flow. Patients are automatically entered in the Cordata system at discharge from emergency rooms, and they can be tracked and receive reminders for follow-up care, medications and other messages. That approach helps ensure that patients don’t “fall through the cracks” and remain engaged during treatment plans that can involve multiple providers and facilities.
