Cincinnati, OH – January 28, 2017: Cordata Healthcare Innovations today announced it has acquired the Clinical Outcomes Report Card System from Prizm Development, Inc. Effective immediately, Cordata will use Prizm’s outcomes system to help its clients analyze resource utilization and reduce the overall cost of their spine care programs.

This analysis, in combination with quality outcomes data collected and reported from the Cordata care coordination and patient engagement platform, enables spine care providers to move quickly in their transition to value-based payment systems. Specifically, providers will be better positioned to maximize MIPS payments as defined in the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA) and better prepared for more transparent and effective direct-to-employer treatment bundles.

The acquisition also includes a consulting partnership under which Prizm will interpret outcomes data and propose improvements for hospitals and health systems to improve clinical and financial performance. In addition, Prizm will act as an independent audit firm for hospitals and health systems wishing to pursue accreditation as a spine center of excellence.
