The updated version of Cordata’s oncology product is easily paired with its complementary product for palliative care specialty care coordination. Cordata has listened to its clients who asked for the ease to co-manage cancer patients who were receiving palliative care in tandem with their treatment or in the form of end-of-life care. The CARES tool is embedded into the Palliative Care product, for ease of managing end of life patients. Also available are Behavioral Health and Pain Management.

Cordata Oncology 7.0 offers some key new features such as a longitudinal view of a patient’s multispecialty oncology care, problem management direct from distress assessment results, patient symptom gathering via a text message and email, dynamic case review support (tumor board), care plans and their supporting workflow steps, and risk stratification for management of patient populations within a tumor team. Also included on the new platform is the live view of AONN+ measures to support operational metrics and national best practice comparison, for care coordination excellence.

Also in the update, Cordata’s patient engagement layer features that were launched in late 2016 have also been enhanced to include deeper connections with patients such as care plan-specific symptom gathering and management. “It is not enough to just reach out to patients to remind them of an appointment or ask for satisfaction ratings. Oncology care needs to anticipate patients’ needs based on their distress, chemo regime or other treatment possible side effects. This is where you can really impact the patient in a life-changing way.” said Cam McClellan Teems, the oncology specialty leader at Cordata.

Another key feature on the new platform is the availability of Care Plans for use in the care coordination activity space. Cordata’s care plans can be from national guidelines, national society practice guidelines, algorithms unique to your Cancer Center – or developed with our pathway partner, Appian360.
