Cincinnati, OH – July 7, 2017: Cordata Healthcare Innovations, a leader in specialty care coordination and patient engagement technology, recently announced a deal with Valley View Hospital in Glenwood Springs, Colorado to implement Cordata’s technology in the hospital’s spine center.
Valley View Hospital will deploy Cordata’s specialty care coordination platform to guide patients throughout their courses of treatment and across the hospital’s comprehensive spine program. The implementation will also enable automated patient follow-ups and the collection of outcomes data as part of Valley View’s strategy to improve the patient experience, drive better outcomes and establish a spine center of excellence.
“We are excited to work with Cordata to enhance the quality of care we deliver to all of our spine center patients,” said Jessica Lorah, Valley View Spine Center’s Administrative Director. “Meaningful behavior change begins with engaging our patients from the moment they walk through the door. With Cordata’s coordination toolsets, we can stay in contact with our patients, including those most at-risk for readmission, along the entire length of the care path to ensure that they keep on track with their recovery plans.”
