Conditions caused by many contributing factors are called complex or multifactorial disorders. Examples are :

  • Patients with two or more chronic conditions
  • Patients who have recently been diagnosed with a new complex or chronic condition
  • Patients who have complex conditions easily impacted by social determinants of health
  • Complex disease patients with co-morbid pain
